Request medical records online.
Attorneys, insurers, employers and patients: submit the request below, email medicalrecords@rehabatwork.com, or fax any clinic. We confirm we hold the records, send an itemized invoice, and release the records as soon as it’s paid — by card online or by check.
Pick whichever is easiest. They all land on the same desk.
Online Fastest
The form below asks for everything we need in one pass — who you are, whose records, which documents, how to deliver — and takes the signed authorization as an upload. Nothing waits on a callback.
Send the request and the signed authorization or subpoena to medicalrecords@rehabatwork.com. One address for all 10 clinics. Include the patient’s full name and date of birth and the clinic where they were treated, if you know it.
Fax or mail
Fax to (301) 838-2041 or to any clinic (directory below). Mail to Corporate Office, 7668 C Standish Pl., Rockville, MD 20855. You can print the form on this page and use it as the cover sheet.
What we need to act on a request
If you are the patient
- Your request — the form below, or an email from you
- A photo ID — upload it or show it at the clinic
- No fee. A copy of your own chart is always free
If you are an attorney, insurer, employer or anyone else
- A HIPAA authorization signed by the patient naming REHAB AT WORK and the records to release, or a subpoena or court order
- The patient’s full name and date of birth, and the clinic if known
- Claim number on a workers’ compensation case — it speeds the search
- Where to deliver and where to send the invoice, if different
Subpoenas and deadlines. Put the return date or hearing date in the “Deadline” field and attach the subpoena. We schedule the release around it.
Fees follow the law of the state where the clinic is.
Maryland and Virginia each set what a provider may charge for copies of medical records. The clinic that holds the chart decides which schedule applies, not the requester’s address. Patients never pay for their own records.
| Maryland clinics | Virginia clinics | |
|---|---|---|
| Preparation / search fee | $22.88 | $25.00 |
| Per page, paper by mail | $0.76 | $0.60 |

Electronic, secure delivery
Paper, by mail
An estimate, not an invoice. We search first; if we hold no records there is no fee. The invoice we send after counting the pages is the amount due. Rates current as of 2026-09-24.
Free by law
- The patient's own chartWe never charge a patient for a copy of their own records, in either state, however it is delivered.
- Social Security disability (Maryland)No fee when the records support an SSDI or SSI claim or appeal and the request comes from the patient, their representative, a nonprofit legal services program, or the patient's own attorney.
- Benefits claims (Virginia)One free electronic copy per calendar year when the records support a Social Security, veterans or needs-based benefit claim. A second copy in the same year is billed at the standard rate.
- The Maryland Workers' Compensation CommissionNo charge when the Commission itself requests records.
- Nothing on fileWe search first. If we hold no records for the person and dates named, there is no fee of any kind.
Two dates matter: when we invoice, and when you pay.
business days from a complete request to your itemized invoice. “Complete” means a valid authorization and enough to identify the patient — the form asks for exactly that.
Records are released the business day your payment posts. Pay by card and that is the day you pay; a mailed check adds the mail time both ways.
callbacks when the request arrives complete. Most delay in a records request is a missing signature, a wrong date of birth, or a fee that has to be quoted by phone. The form removes all three.
Two ways to pay. Card is faster.
Pay by card online
Have your invoice number and the amount ready. The payment page is hosted by our card processor — your card details never pass through this website.

Pay by check

Need the records for a referral instead?
If you are sending us a patient, don’t request records — refer. Prior records can follow the referral.
