What happens after you refer
Case managers plan around predictability. Here's the path a referral takes — and what lands on your desk at every step.
-
You → us
The referral arrives
Online form, uploaded script, fax or phone. Missing records or a job description don't hold it up — send what you have.
-
Us → the patient
We contact and schedule
The patient is scheduled at the clinic nearest them — 10 clinics across Maryland and Virginia, serving the DC metro, bilingual at every one. You're told when.
-
At the clinic
Evaluation first, always
Every path starts with a standardized evaluation — therapy eval, program entrance evaluation, or the FCE itself — so there's a baseline number before there's a plan.
-
Us → everyone · within 72 hours
The report goes out
FCE reports are sent within 72 hours of the evaluation, to every party you listed — adjuster, physician, nurse case manager, attorney, employer. Capabilities stated in job terms, not chart terms.
-
Ongoing
Treatment with a scoreboard
If therapy or a work program follows, you get regular progress reports against job-specific goals — and an immediate call on attendance or participation problems, not a surprise in week four.
-
Discharge
A recommendation you can act on
Discharge documentation shows measured functional change, entrance to exit, and states whether the patient meets the demands of their own job. If the goal is settlement or closure, the documentation serves that decision instead.
-
After
We stand behind the paper
Peer review and expert witness services are available when a case needs the clinician behind the report — plaintiff or defense.
